3. PSA Birth Certificate (original copy)

4. Certification of Completion 

5. Certification of Current Enrolment

6. Certification of Good Conduct 

7. Report Card/s from school attended prior to the incoming school year (original copy)

8. Duly accomplished and signed Data Privacy Policy Agreement (original)

9.  Interview
